The term 'context' is vague and overloaded. Its usage for 'an execution
context' is now referred as such, in both comments and some APIs' names.
When the execution context can only be a fiber or a task (i.e. not an
ISR), it is referred to as a 'thread', again in comments and everywhere
in the code.
APIs that had their names changed:
- nano_context_id_t is now nano_thread_id_t
- context_self_get() is now sys_thread_self_get()
- context_type_get() is now sys_execution_context_type_get()
- context_custom_data_set/get() are now
sys_thread_custom_data_set/get()
The 'context' prefix namespace does not have to be reserved by the
kernel anymore.
The Context Control Structure (CCS) data structure is now the Thread
Control Structure (TCS):
- struct ccs is now struct tcs
- tCCS is now tTCS

The initialization procedures need to be called in the
following order:
- basic hardware initialization;
- devices initialization;
- hardware clock initialization, that the kernel needs to
run the kernel server.
This way, the board initialization routines is placed
to the pure_early_init level to make sure it runs first,
all device initialization procedures run at pure_init level,
so they are initialized early enough to be used for debugging
if needed. Hardware clock initialization is placed on
nano_early_init level to make sure hardware clock is initialized
just before the kernel server starts.

With this, the UART console has its own initialization which is not
dependent on UART init routines.
This will allow the app to utilize all UARTs for other purposes,
instead of always reserving one for console. Future patch
will enable this choice.
Note that the console init is effectively demoted to pure_init
from pure_early_init. The UART console depends on UART being
initialized, but there is no deferred init at this point.
So this initializes the console a bit later than UARTs,
simply to make sure the UART is initialized before using it.
Once there is a mechanism for some type of deferred initialization,
the UART console init can be promoted back to pure_early.

This restructures the device structs and now utilizes the new
driver initialization model. This is another step towards
converting the serial driver to the new driver model.
Note that the serial driver does not initialize the hardware
unless it is being used by another driver. The configuration
of the serial port needs to be done by the driver utilizing
the port (e.g. baud rate, interrupt priority, etc.).
Therefore, some serial ports are declared but not exactly
configured.
Also note that the UART console is being initialized at
the same time as the serial port. This will be removed
in future patch, so the UART console driver will do
its own initialization.

This change removes the internal number-to-function mapping
of microkernel services. Instead, function pointers are used
to specify which service to use.
This is in preparation for private kernel objects. Before this,
only kernel objects that are defined in MDEF files would have
corresponding functions included in the final binary, via sysgen
by populating an array of number-to-function mapping. This
causes an issue when a certain type of objects are all defined
with source code, and never in MDEF file. The corresponding
mapping would be deleted, and the functions are never included
in the binary. For example, if no mutexes are defined in MDEF
file, the _k_mutex_*() functions would not be included.
With this change, any usage of private kernel objects will hint
to the linker that those functions are needed, and should not be
removed from final binary.

IRQ_CONNECT_STATIC() macro provides static interrupt registration.
It creates an entry for _sw_isr_table, table of interrupt handlers.
IRQ_CONFIG() macro provides interrupt controller configuration.
On ARM platform it configures the priority of each interrupt.
_sw_isr_table is implemented the way that each of it's elements
is located in individual section. Sections are marked a "linkonce",
thus when a device driver declares an interrupt handler, it overwrites
the default _sw_isr_table entry.

This driver is a very simple UART driver that can be used to implement
a protocol in application. It has no protocol logic and just allow
application to receive and send data over UART in platform independent
manner.
API is designed to minimize number of required memory copies.
Application is able to provide new buffer every time it decides to
consume received data. It is also able to just alter offset and
reuse current buffer eg if data was corrupted or otherwise invalid.

Since the address of the new context is known before _NewContext() is invoked
(due to it being passed a properly aligned stack), there is no longer any
need for _NewContext to return the pointer to the context.
Furthermore, as a direct result of the properly aligned stack, the pointer to
the new context does not need to be passed as a separate parameter since it
will always match the passed stack pointer.

- It fixes the include paths in Makefiles.
- It fixes the include path for the offsets.c compilation.
- It fixes the linker command. This changes is needed because gcc
ARM cross-compiler does not accept an AT() command without
specifing an address explicitly. This corner case appears
on ARM architectures when XIP is disabled.

Renames the include file defining non-public microkernel structures
from k_struct.h to kernel_structures.h, and relocates it to the
microkernel's non-public include file directory. This means that
applications and drivers including the microkernel's public APIs,
using microkernel.h, can no longer access non-public information.
Note: This change also eliminates some redundant #includes by the
microkernel's own subsystems, since the inclusion of minik.h brings
in the vast majority of public and non-public APIs.

inline assembler code in a set of header files.
If another compiler is used, that supports a different
inline assembler format, asm_inline_<compiler name>.h
heder files have to be provided.

Moves all files in this sub-directory to kernel/nanokernel, then
deletes the sub-directory itself. Both of the relocated files are
associated with fundamental nanokernel capabilities, which makes
their inclusion in the main nanokernel source directory logical.

Moves all files in this sub-directory up one level, to the main
nanokernel source directory, then deletes the sub-directory itself.
This change means that all nanokernel source files (with the
excecption of its include files) are now in the main nanokernel
source directory, which is a more intuitive arrangement.

Relocates start_task() so that it appears in the main microkernel
directory, alongside its complementary routine abort_task();
this corrects a long-standing anomaly in which this microkernel-
specific routine appeared in the nanokernel portion of the tree.
With this move, the start_task.c file and its parent nanokernel/task
sub-directory no longer serve any purpose and are removed. (Note that
no changes are required for ARC architecture support, which does not
support microkernel capabilities and doesn't use this sub-directory.)

Since multi-node systems are not supported, the 'Srce' field now only serves
one purpose--to indicate whether the packet was allocated from the command
packet pool. As such, a better name (and type) for it is 'alloc' (of type
